Oversees and manages all aspects of client needs. Our service embraces medical needs but is not limited to just that. We also can assist with transportation, socialization, household and car repairs.
Complete Assessment with Report...
Provides clients with a report identifying and evaluating their current health, functional, psychosocial and safety needs. Care managers identify potential problems to prevent incidence of crisis, which allows clients to maintain or improve their current level of functioning.
Placement Assistance...
Provides the client with a brief assessment of level of care needs which includes a review of current situation. An in depth report of appropriate facilities is prepared and an experienced RN visits each potential facility for accuracy.
Medical Record Review...
An independent review of all available medical records is generated from various providers by an experienced RN. This assures accuracy and enhances coordination of care.
Welcome Home Services...
Provides assistance to seasonal clients to assure that their home is ready for their return. We also make medical appointments in advance of their return.
Hurricane Preparedness…
Ensure client has adequate supplies and makes hotel reservations if necessary. Reviews evacuation plan with family and reports status after a storm. For more information on Hurricane Plan click here.
Special Needs Trust Monitoring…
Provides a detailed report with recommendations for appropriate equipment and service needs for elderly and pediatric clients.
Covid-19 Risk Assessment…
Assess clients Covid risk by identifying high risk activities and lifestyle choices. Provide clients with instruction on how to properly wear a mask and use Covid-19 protocols to keep them safe at home or when going out.
